Since the start of 2024, some of the larger private insurance companies are now requiring that for patients to receive reimbursement for out-of-network care, their practitioner's signed W-9 Form must be included along with the claim(s).


Reimbursify handles the process of including a signed W-9 Form when filing claims, if we have it on file.


Follow the steps below to upload your signed W-9 Form via the Reimbursify Practice Portal.


Click here to generate a W-9 Form that you can sign in 2 minutes.


Step 1:

Navigate to the My Practices > Addresses page of the Practice Portal




Step 2: 

For each Practice Visit Address, click the "Edit" option.



Step 3:

Click the "Upload" button in the W-9 Form section. Locate the file on your hard drive and submit it.




Step 4:

Click the "Save" option.



Repeat the steps above for each Practice Visit Address, even if the same W-9 Form is used for additional addresses.


NOTE: If you have multiple Practice Visit Addresses, and there are different Tax ID's associated with them, you will need to provide a unique W-9 for for each address/Tax ID combination.
