Follow the steps below to register as an out-of-network practitioner with BCBS of Louisiana:

Please note: The process can take up to 90 business days. 

For individuals:

1. Submit a credentialing application using Professional Initial Credentialing Packet. 

For groups:

1. Enroll the group by submitting the Provider Update Request form. 

Once the group has been registered, each practitioner should submit a credentialing application using the Professional Initial Credentialing Packet.
