Follow the steps below to register as an out-of-network practitioner with BCBS of Illinois:

2. Start a new provider enrollment form

3. On the first step of the provider enrollment application, select “Participate out-of-network”

4. You will be prompted to download and complete a roster to complete and submit with your application

5. Follow the prompts to complete the registration process and submit.
