Follow the instructions below to file a claim for a minor (dependent) via the Practice Portal.


1. Click the "File a Claim" button on the Claims page of the Practice Portal.



2. Enter the parent's email address when prompted.




3. On the next screen, enter the parent's information in the top section.

4. Select "NO" when asked, "IS THE PERSON ABOVE THE INSURED MEMBER FOR WHOM THIS REIMBURSEMENT IS BEING FILED?"

5. Select "YES" when asked, "IS THE PERSON ABOVE THE PRIMARY POLICY HOLDER?"

6. Enter the minor's information in the section labeled "WHO IS THIS REIMBURSEMENT FOR?"



7. When prompted, select "YES" to verify that you are "an authorized representative of the health care practitioner/organization listed on this claim."



8. Complete the rest of the practitioner and visit information on the claim form as you would with a non-minor.
