Follow the instructions below to file a claim for a minor (dependent) via the Practice Portal.
1. Click the "File a Claim" button on the Claims page of the Practice Portal.
2. Enter the child's information in the "Client Information" section, and the parent's email address in the "Reimbursify App User Information" section.
3. Follow these instructions for completing the rest of the form.
